Upgrade the `bnf2html.pl` script to spot repeated rule definitions. Update `sql-2003-2.bnf` and `sql-99.bnf` to eliminate various repeated rules. The repeats in SQL-99 mostly come from extra information in 9075-4 (SQL/PSM) and 9075-5 (SQL/Bindings). The repeat in SQL-2003-2 was similar. Also, update contact information.
