User registration

This is related to #51
I have added a block on the user registration page http://hub.kendra.org/user/register called Account application Info with the following text:
By submitting the form you will create an account request that will need to be authorized by one of the site administrators.

I think we need more information that explains what type of users should apply for an account and possibly add fields to the form to allow users to pass in info that will allow us to identify if a user should be approved.

It would be good to at least take the same information as Kendra Home. To be honest at this stage I'll be adding people manually after speaking to them. I'd like them to have a fully set up profile on Kendra Home before we add them so not sure how to identify that.
